How do I find out if you accept my insurance?

Please fill out the form at the bottom of this site and/or send me an email with your insurance details and I will use the verification tool on Simple Practice to verify your insurance eligibility. Insurance must be verified prior to scheduling (unless you are utilizing cash pay).

I am working to be in network with most major insurances in Washington State and will be updating this page as contracts come through. I am currently approved with the following:

I do accept cash payments. If you are unable to afford services please feel free to reach out about a financial hardship waiver.

Interactive Complexity Add On (90785) $ 25

Crisis Therapy (90839) (30-75 minutes) $195

Crisis Therapy additional 30 min each $ $70

Brief Emotional/Behavioral Assessment (96127) $15.00 – per unit (test)

Forms and letters outside of appointment $50/hour, billed in increments of 15 min.

Letters for attorneys billed at separate rate $50/hour

Electronic Records Fee (email, CD) $6.50/flat rate

Editing of confidential information, per WAC $6.50/hour

No show or late cancel fee for follow-up clinic visits Equivalent to your appointment Charge

Clerical fee for searching/handling records, per WAC $28.00

Pages 1-30 (copying fee), per WAC $1.24 per page

Pages 31+ (copying fee), per WAC $0.94 per page
